ElderBridge SoCal serves seniors living in and around the Southern California cities of Claremont, La Verne, Glendora, Upland, Rancho Cucamonga, Glendora, San Dimas, Walnut, Diamond Bar, Chino Hills and Ontario.

Owner, Sharon Sparks, is a registered nurse who believes that EVERY senior over 70 should have a caring, knowledgeable advocate at their side whenever they attend important doctor appointments or when they are hospitalized. She knows how easy it is for elders to downplay the severity of symptoms, misunderstand, or totally forget what their doctors tell them. She has heard the stories of patients being released from the hospital only to have to return in a few days because they did not follow through with discharge instructions. Sharon and her team of Partners in Care are passionate about thorough communication, so her clients tell their doctors all they should, and doctors attend to the unique preferences and needs of her ElderBridge SoCal clients.

Everyone needs to be nurtured, from the time they are born to the time they pass, and most adults are good at nurturing themselves. But many elderly seniors living alone have no close family or friends who can help them as they age, and they no longer drive. Without intervention during a crisis, they will decline physically, cognitively, emotionally and/or spiritually. ElderBridge SoCal not only stops the decline, we nurture vibrant life, leading to longer independence.

Many adult children of seniors work and live far away from their parents. Some can take time off work to handle the health crisis their mother or father suffers, but they must return to their busy lives. ElderBridge Partners in Care make sure their elderly clients follow through with all steps leading to health, and gift their families with peace of mind.

ElderBridge services has been helping our family out with our dad who has Dementia for over a year now. I live in Arizona and my brother in Missouri which makes it difficult in dealing with our fathers daily needs. Our dad has lived in the same area for over 50 years and knows the area, his surroundings and the comfort of his own home. For my brother and I to find a service like ElderBridge has been a complete blessing for us. Not only is their staff knowledgeable in dealing with Dementia which can be very changeling at times, but the staff is very friendly like part of the family. Everyday we get a report of the visit and fun day they had, like you were right there watching their day. Our father looks forward to their visit’s daily as he is dressed and waiting from them to arrive. Because our father can not drive anymore, they take him shopping or to the local senior center for activities for the day, they take him to get his hair cut or go get something to eat. Some days it’s just enjoying a football or golf game on TV and on Sundays they take him to church, whatever our father would like to do for that day. I would highly recommend ElderBridge services to anyone that needs a week away or just some help with a loved one. They are a complete blessing to our family, or an addition.
