Patient Advocacy and Geriatric Care Management: A solution based approach to navigating you and your loved one's through the health and senior care system.

Initial individual assessment to determine care and needs.

Develop and implement a personal care plan to improve quality of life and retain independence for as long as possible, finding answers to the challenges they are facing.

Ongoing care management, reassessing the plan as abilities and needs change.

Obtain and oversee home health agencies, social and rehabilitation services.

Communicate and coordinate with other professionals, service providers and family members; provide trusted referrals within the community

Collect all medical records, coordinate medical appointments, confer with and facilitate communication among all the physicians, caregivers, home health agencies and hospitals, nursing homes, assisted living and /or rehabilitation facility.

Research and assist in transitions from home to assisted living, senior communities and skilled nursing facilities, where we will continue to monitor and oversee care.

When hospitalized our experienced patient advocates guide the patient through the healthcare system, ensuring the diagnosis, prognosis and treatment options are clearly understood.

Research illness and disease, conferring  the education needed to give true “informed consent”

Ensure patient centered care, by facilitating and managing physician care conferences, keeping all the caregivers are on the same page and treating the whole person, not only the doctor’s specialty.

Manage discharge plan and refer to appropriate rehabilitation facility. Oversee care plan at rehabilitation facility and manage transition.

Evaluate home and arrange for home care, nursing, therapist, aides, equipment, then manage and monitor the care and oversee the transition.

When Carol's pain was intolerable and we were absolutely exhausted, it seemed like all we hit were dead ends until we met Jackie. she was an angel disguised as our health care advocate in our desperate last months of need. Oh! How i wish we found her so much earlier. Jackie managed all aspects of Carol's care getting us nurses and health aids over the holidays when hospice claimed they had no one. She made things happen, when we had no place else to go, so all we needed to do was be there for Carol, in her last hours. Jackie was truly a blessing.

Although we were eventually very satisfied with the home health aids, nurses, equipment and supplies provided, it was only after your intervention on our behalf that timely and complete services became the norm. Jackie O'Doherty helped out during a very stressful period for me and my daughter. When my daughter was diagnosed with thyroid tumors after a routine physical, we turned to Jackie for help in finding a pediatric endocrinologist -- not an easy proposition. Within a day, she had located an excellent doctor, near our home, who proved to be very helpful and thorough. He ultimately referred us to one of the nation's top pediatric endocrinologists for a biopsy and confirmation of his diagnosis. This physician was at the Yale Pediatric Specialty Center, and Jackie even helped us with planning the best route for our drive there, to New Haven,CT from our home in New Jersey. She made a stressful time much more manageable, and we are very grateful.

Jacqueline O'Doherty of Health Care Connect came to my rescue. After my husband’s diagnosis, I had tried to navigate our health insurance coverage by myself. I spoke for more than two months with five or six different “oncology nurse specialists” who rarely returned my calls and could not answer my simple questions about benefits for my husband’s bone marrow transplant. When I did speak with a nurse, she misinformed me about his hospital’s coverage and told me we were foolish to use a hospital other than the one she suggested. Meanwhile, my husband was going through treatment and needed my care. Then I found Jackie! In a few hours, she was able to resolve my serious conflict. Jackie spoke with my husband’s doctor, hospital, benefits manager, and insurer. She got me the answers that I was unable to accomplish on my own. She is supportive, professional, thorough, and really knows the health care system. She gave me the peace of mind that financially we were going to be okay so that I was able to focus on getting my husband well again. Thank you Jackie for your valuable service.

I consider myself lucky the day I was referred to Jacqueline.
