  • Coordinate and attend medical appointments to enhance communication and improve patient’s understanding of plan of care. Summarize visit in concise format and communicate with family.

  • Interview and assess Aides and LPNs being considered for home care.
  • Compile comprehensive list of needs for consideration in Medicaid Conflict Free evaluations to ensure maximum level of coverage is considered.
  • Implement comprehensive home health care plan, identifying Companions and/or Aides to enable clients to safely age in place. Coordinate home safety assessments.
  • Provide daily or periodic home well-being check ins.
  • Coordinate Skilled Care in the home through Medicare, including nurse visits, physical, occupational and speech therapy.


  • Obtain improved quality of life for patients by partnering with social workers, nursing staff, aides, and therapists. Examples of outcomes include:
  • Successfully challenged denial of ongoing physical and occupational therapy
  • Identified patient over-medication due to recent weight loss, resulting in improved alertness and engagement
  • Challenged nursing staff’s assessment of patient recovering from heart surgery, resulting in additional testing that identified a brain hemorrhage that was then successfully managed
  • Successfully challenged premature discharge of patient from hospitals and nursing facilities, obtaining additional specialized treatment including palliative and hospice
  • Facilitate discussions relating to Assisted Living options, oversee move and assimilation into Assisted Living.
