Longboat Health Advocates is very experienced in not only the acute care hospital setting but is also very knowledgeable in post acute hospital care placement whether  in an acute care rehabilitation or short term skilled rehabilitation unit.

We provide a proactive “Emergent” planning to be prepared for the possibility of difficult times for individuals who might live alone or live in another demographic location than their family members.  Knowing there is a support team in place to ensure our clients get the best possible healthcare in the event of an emergency provides peace of mind for both our clients and their family.

We  have a strong knowledge base for placement based on  the individual needs of those patients with Alzheimer’s or Dementia that require  a  Memory Care  Unit.

In addition to  placement help for acute or skilled rehabilitation we are also well versed in placement of all levels of care from Assisted Living to Long Term Care needs.

We also recognize that  difficult medical or end of life decisions can be taxing on family relationships and Longboat Health Advocates can provide family mediation in these trying times.

There are not enough stars to give to Paula. Where to begin. My 101 yr old mother in law who I knew my entire life was unable to talk or get out of her bed for 6 months ( living independently) with 24 hr care giving and hospice care . I was her advocate and took care of everything. It became overwhelming and that led to us finding Paula . I cannot express how amazing she is . She was my advocate as well as mom’s and helped us navigate through this traumatic process till the end . She was available 24/7, wiped tears , held hands and helped us so much, she was always happy to talk to us. A wonderful communicator . She was so kind to the aides and our extended family . I wouldn’t have been able to cope without Paula . I give all my thanks and appreciation to her .

My sisters and I were at a loss trying to navigate the health system in Bradenton and we were very fortunate to meet Paula Miller and enlist her services in dealing with our sister who had very serious health issues. Paula was very professional, clear, very easy to understand and deal with, and at the same time very friendly and compassionate. There was an immediate sense of relief terms of managing the process, and she quickly and seemingly easily helped us sort out our priorities, develop a plan and dealt with the health care providers in executing the plan. There are not enough stars or words I can type to let you know how wonderful, compassionate, pragmatic, and empathetic Paula is. She did so much for our family in a very short time. As soon as I was lucky enough to find her, she immediately took over every aspect of communication with the facility. Her extensive experience in the field combined with her compassion and empathy is a combination very few have and exactly what our family needed as we struggled with the medical aspect as well as the emotional issues related to a loved one in the situation that had quickly developed and was difficult for us to grasp. Paula held our hands from our first meeting with her until my father took his last breath. Her support on every level was simply a gift and we will never be able to repay Paula for what she did for our family.

During a benefits dispute, when I was on ‘frustration overload’, Ms. Miller was able to step in and work with my provider and insurance company to determine what my rights were and to establish a plan forward for me. She clearly explained the entire situation, including my rights as a patient, my insurance benefits and the options available to me.
Ms. Miller is passionate about ensuring that her clients get the best care to which they are entitled.
