The scope of my advocacy practice is ever evolving.  Over the past 4 years I have assisted over 50 clients to make difficult decisions regarding their personal health, placement options, qualifying for Medicaid, and guidance to appropriate community resources. Services include:

  1. Accompany the client to medical appointments for the purpose of asking   appropriate questions and interpreting medical language.
  2. Interpret lab reports, medical diagnoses, medical treatments and protocols to patient and family members.
  3. Evaluate the status of all therapies (mental, physical, and occupational).
  4. Visit patient in the hospital and communicate with hospitalist, medical specialists or other staff as needed to coordinate therapies to ensure the desired outcome is in the best interest of the patient and the family.
  5. Review hospital medical records for aiding family in understanding their options and decisions to be made.
  6. Develop a personalized, detailed care plan for each client centered around the medical diagnoses and the wishes of the patient and family.
  7. Work with family M.D. and Nurse Practitioner to assure referrals and orders are properly/timely placed.Develop good working relationship with care coordinators at each hospital in the Tidewater area as well as at long term care facilities and assisted living establishments. Overcome obstacles.
  1. Help family to find valuable community resources for socialization, transportation, legal assistance.
  2. Explore and respect family’s wishes for desired quality of life at the end of life and develop a formal plan to ensure proper administration.
  3. Provide community resources to family and facilitate the patient’s transition if independent living is no longer an option
  4. We always maintain suitable access via phone and e-mail to support a patient’s needs and to ensure their Care Plan is administered in a timely fashion.
  5. A Home safety assessment is typically provided to advise and advocate for home safety.
